CALIFORNIA CONSUMER PRIVACY ACT PUBLIC FORM

This form allows California residents to submit requests under the California Consumer Privacy Act (CCPA), as described in Quontic Bank’s California Consumer Privacy Notice. Consumers may use the form to submit:

  • Right to Know requests (asking what personal information we have collected), and
  • Right to Delete requests (asking us to delete certain personal information).

The form collects the information needed to:

  1. Verify the identity of the person submitting the request,
  2. Confirm their relationship with Quontic Bank,
  3. Determine whether any exemptions apply, and
  4. Document the request for compliance and record‑keeping.

All required fields (*) must be included so we can verify the requestor. Information collected is used only for identity verification and compliance with the CCPA.

Once the form is submitted, Quontic Bank will:

  • Send confirmation within 10 business days, and
  • Provide a full response within 45 days (or up to 90 days if an extension is needed).

We may contact the consumer if more information is required.

By providing your phone number, you’re giving Quontic permission to use it to call and/or text message you regarding our products and services. While we won’t charge you for the calls or text messages, your service provider may depending on your plan.
